Lipoedema Self-Management Course
What is Lipoedema?
Lipoedema (also spelled lipedema) is a chronic, progressive soft tissue disorder that primarily affects women. It is characterized by an abnormal, symmetrical accumulation of fat, most commonly in the legs, and sometimes the arms, while the feet and hands remain unaffected.
It is not caused by obesity, and it does not respond normally to diet and exercise.
What Happens in the Body?
Lipoedema involves:
- Abnormal fat cell enlargement (hypertrophy)
- Increased capillary fragility → easy bruising
- Chronic low-grade inflammation
- Painful, tender subcutaneous tissue
- Progressive tissue fibrosis (in later stages)
Over time, the altered fat tissue can impair lymphatic function, potentially leading to secondary lymphedema (often called lipo-lymphedema).
Key Clinical Features
- Symmetrical enlargement of legs (hips → ankles)
- Sparing of the feet (the “cuff sign” at the ankles)
- Painful or tender fat
- Easy bruising
- Minimal response to calorie restriction
- Feeling of heaviness
Lipoedema - Summary of the 5 Stages
Lipoedema progresses in stages:
Stage 1: Smooth skin surface, enlarged subcutaneous fat layer, tissue soft and evenly thickened
Stage 1.5: Skin still mostly smooth, small nodules begin to appear in the fat tissue, tissue may feel grainy on palpation
Stage 2: Uneven skin surface, larger nodules in the fat tissue, “Mattress-like” or orange-peel texture
Stage 2.5: Early formation of lobules (small fat protrusions), tissue becoming denser and more fibrotic, mild skin folds may appear
Stage 3: Large lobular fat deposits, significant limb contour deformities, skin folds and advanced tissue changes
*** Note: These stages describe structural changes in the tissue, not necessarily symptom severity.
Who Gets Lipoedema?
- Almost exclusively women
- Often begins or worsens at hormonal transitions: Puberty, Pregnancy, Perimenopause
- Can be triggered by other conditions like leaky gut and histamine intolerance
- Often a family history is present
